Worker dies in blender: Man falls into operating meat blender, A worker died in a blender while working a meat distribution plant in Clackamas, Ore. Saturday. KGW reports April 29 that 41-year-old Hugo Avalos Chanon was the employee killed.

The horrible incident happened at Interstate Meat Distributors. Police were alerted to the employee's death around midnight Saturday. Workers with the DCS Sanitation Management were reportedly cleaning equipment when Chanon fell into the meat blender that was fully operating. Apparently a worker noticed something was wrong and hit the emergency switch to shut off the blender. It was too late. Chanon had died.

Law enforcement and the Occupational Safety and Health Administration launched an investigation.

OSHA spokeswoman, Melanie Mesaros, said Interstate Meat Distributors was cited for multiple safety violations between October 2012 and January 2013. One violation read, "Points of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s)."
